SUMMARY

The purpose of the Juvenile Justice Case Manager position is to provide assessment, counseling and service linkage to court-involved youth, who are at risk to recidivate. This position provides case management for individual and group sessions, advocacy, life skills training and educational group activities.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:

  1. Maintain an active caseload of up to 5-10 clients.
  2. Enter client information into Orbis Youth Assessment Screening Instrument (YASI) system within 72 hours of intake
  3. Collaborate and network with law enforcement, probation, employers, family members, schools, health providers and any other providers involved in the client’s service planning.
  4. Create and maintain Individual Case Plan for all youth involved in program.
  5. Execute routes in either an agency vehicle or one’s personal vehicle. This may include preparing the agency vehicles for daily use (I.e. filling up gas.)
  6. Assist in the planning of cultural, educational and/or social outings, for the purposes of the individual case plan.
  7. Serve as group facilitator/co-facilitator for evidence-based curriculums.
  8. Address individual youth and group behaviors using motivational interviewing and de-escalation skills.
  9. Provide information to the Program Coordinator and Juvenile Justice clinician relevant to any unaddressed, perceived client needs as part of on-going assessment of referred youth.
  10. Ensure safety and well-being of clients during all programming hours.
  11. Assist in driving and maintaining vans, transporting clients to and from program sites, transporting food to program sites.
  12. Complete all required paperwork in a timely and accurate manner.
  13. Complete incident reports regarding client behaviors and other program related incidents before 9 am the following morning.
  14. Ensure programming is implemented within the designated hours.
  15. Prepare and submit all paperwork accurately and within the prescribed timeline.
  16. Maintain Food Handlers Certificate for basic food handling.
  17. Complete all Core Curriculum training(s) required for this position. Complete on-going training as designated by program requirements.

WORK HOURS

Willing to work varying hours based on the need of the program. Hours may include evenings and weekends at any of the AMHW Reporting Centers.

CARE COORDINATION

Individuals in this position may have a leadership or shared role in coordinating care for clients within the program and a specific geographic area.

QUALIFICATIONS REQUIRED

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

REQUIRED EDUCATION and EXPERIENCE

Bachelor’s degree in human services or related field required. Previous experience working with at risk/ juvenile justice involved youth preferred.
