Job Description
POSITION SUMMARY The LVN will report to the Wellness Director and will assist by helping manage the Wellness Department and prioritizing resident health, including but not limited to overseeing medication administration and record keeping, scheduling staff, developing daily tasks, initiating, and maintaining emergency packets, and will ensure the organization of resident charts.
ESSENTIAL DUTIES AND RESPONSIBILITIES In addition to all components of the above position summary the following "Essential Duties and Responsibilities" are essential to successful performance in this position
- Monitor team members' medication administration, including frequent audits of medication records, order medications from pharmacy, maintain record, and assure that medications are properly stored and cared for.
- Records all significant changes regarding resident health and mainstream communication to the appropriate parties. This includes residents' well-being and any reactions to drugs, treatments, and significant incidents.
- Assist with medication management as needed, including med pass and documentation.
- Update medical charts and care plan. Maintain daily resident records in accordance with company policy and procedures. This includes documentation related to medication distribution, leisure activities, and incidents/observations.
- Perform work assignments and attend required in service training. Report concerns and
- Incorporate State Department of Social/community Services/Public Welfare/Licensure requirements during in-service training and orientation.
- Review and implement incident reports; evaluate for need of intervention to minimize risk of further occurrences.
- Schedule Personal Care Assistants and develop daily tasks and make assignments.
- Ensure medical supplies are always in stock. Takes inventory and orders supplies. *
- Follow up weekly on bath schedules and vital signs (blood pressure, weight, and respiration, etc.).
- Act as point of contact during shift for communication with supervisors, doctors, and families.
- Contribute to Community relationships by demonstrating cooperation and professional conduct with residents, families, fellow team members, physicians, pharmacies and other vendors or persons.
- Contribute to team effort by being flexible in work assignments; by furnishing support, taking initiative, and by understanding how this position affects and compliments all other community positions.
- Perform activities on schedule while maintaining regular attendance and punctuality within specified tolerance.
- Maintain an appropriate work pace to the given workload.
- Maintain all health equipment for safety and proper function.
- Lift and/or transfer residents as needed.
- Correct environmental hazards in an appropriate and safe manner.
- Check resident files weekly for appropriate and necessary signatures.
SUPERVISORY RESPONSIBILITIES - The incumbent in this position will have no direct reports.
EDUCATION/EXPERIENCE/CREDENTIALS - Valid and unencumbered California LVN license
- This position requires a high school diploma or equivalent certificate and prior care experience in an assisted living community or skilled nursing facility.
- Mandatory Live-Scan Clearance, TB test, physical exam and CPR/AED certification are required upon job offer.
- Preferred certified nursing assistant certification.
- Able to speak, read, write, and understand the English language fluently.
- Ability to work additional hours which could include evenings and weekends.

- Familiarity with relevant healthcare regulations and compliance requirements for assisted living facilities.
- Knowledge of designing, implementing, and evaluating wellness programs tailored to the needs of elderly residents.
- Awareness of health and safety protocols to ensure the well-being of residents and staff.
- Understanding emergency protocols and procedures to handle crisis situations effectively.
- Knowledge of medication administration, storage, and adherence practices
- Effective communication with residents, their families, staff, and other healthcare professionals
- Ability to identify challenges and find practical solutions to enhance resident well-being and program effectiveness.
- Ability to manage multiple tasks, scheduling, and maintaining records efficiently.
- Ability in utilizing software and technology for program management, documentation, and communication.
- Being flexible and adaptable in a dynamic healthcare environment to meet changing resident needs.
- Ability to Conduct wellness assessments and individualized care plans for residents.
- Working effectively with interdisciplinary teams, including nurses, therapists, and administrators.

- Willingness to stay updated on industry trends and best practices in elderly care and wellness programs.
