Job Description
Qualifications: Must be a registered nurse currently licensed to practice in the state of Wisconsin. A minimum of three years experience in nursing is required. Experience in long term care management is preferred. Must be able to assess resident conditions and function in multiple priority situations. Must have knowledge of governmental regulations and possess leadership ability and effective communication skills.
Status: The Nurse Manager is directly responsible to the Director of Nursing.
The Nurse Manager supervises the nurses and nursing assistants.
Essential Job Functions:
Assesses, plans, implements, and evaluates the overall care of the residents according to state/federal guidelines
Oversees day-to-day nursing care performed by nurses and certified nursing assistants to ensure that quality care is maintained at all times
Supervises all nursing personnel.
Specific Job Duties:
Coordinates continuity of care for all residents
Ensures that a comprehensive care plan has been established and maintained for each resident
Completes MDS paperwork as scheduled to meet state/federal guidelines
Oversees wound care assessments and management
Ensures that residents are admitted, discharged, and transferred according to facility policies
Completes rounds for the skilled nursing to ensure all cares are being performed adequately
Monitors resident conditions and contacts appropriate persons if there are changes in condition
Participates in resident care conferences on a weekly basis
Reports all pertinent information to the preceding and proceeding shifts
Works collaboratively with the nursing team and nursing assistants. On call to assist nursing staff as needed with resident concerns including assessment, documentation, and staffing.
Maintains confidentiality of all resident care information
Promotes positive interpersonal relationships with residents, families, and team members
Reviews complaints/grievances made by residents, staff, and families and assists with solving issues
Monitors medication passes and treatment schedules to assure medications are administered correctly
Administers medications and treatments as deemed necessary
Ensures physician and lab orders are transcribed in a timely manner
Ensures that weekly, Medicare, and conditional charting has been completed
Authorizes the use of alarms and specialty mattresses when necessary
Obtains appropriate consents from residents, families, and/or guardians when necessary
Oversees management of BIMP’s, psychotropic use, TD screenings and sleep assessments for hypnotics
Oversees the management of scheduling physician, dentist, and podiatrist appointments
Acts as a liaison to staff, residents, and families
Acts as a role model to nursing staff by teaching, demonstrating, and orienting staff as necessary
Participates in evaluating, auditing, coaching, and counseling of nursing personnel
Follows facility Infection Control guidelines as appropriate
Works cooperatively with rehabilitative services and applies knowledge of rehab techniques
Keeps the D.O.N informed of significant resident and staff changes
Keeps abreast of current nursing practices and trends through individual study
Works with Staffing Coordinator and assists with scheduling needs for all shifts as necessary
Adheres to nursing supervisor performance standards as outlined by the Nurse Practice Act
Attends scheduled meetings and in-services on a regular basis
Participates as a member of the Nurse Management team and Resident Care committee
Participates in the implementation of the facility’s quality assurance program
Works scheduled hours complying with facility attendance policies
Demonstrates knowledge of, and adheres to, actions to assure resident and staff safety
Practices good personal hygiene and facility dress code
Performs other duties as assigned by supervisor
Acts as a back up to the Assisted Living/ Independent Living Nurse Manager
