We are seeking an experienced Registered Nurse (RN) to conduct home visit assessments for adults and pediatric individuals applying for Medicaid Long-Term Services and Supports (LTSS). The RN will complete comprehensive in-home evaluations to determine eligibility for Medicaid-covered personal care, respite, and other home- and community-based services.

Responsibilities

• Perform face-to-face home visit assessments for applicants, including adults and children.
• Complete LTSS screening tools and documentation accurately and in accordance with state and Medicaid guidelines.
• Communicate findings to the care coordination team and applicable agencies.
• Maintain timely, clear, and compliant documentation.

Preferred Experience

• Prior work with DMAS LTSS
• Background in home health, case management, or LTSS screenings strongly preferred.

Requirements

• Active VA RN license in good standing.
• Reliable personal transportation for travel to home visits.
• Strong assessment, communication, and documentation skills.

Training & Work Environment

Once fully oriented and working independently, the RN may complete most responsibilities remotely, aside from required in-person home assessments.
