*Overview*
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will be responsible for coordinating patient care, collaborating with healthcare professionals, and ensuring the delivery of high-quality services.

*Responsibilities*
- Utilize medical expertise to assess patient needs and develop individualized care plans
- Coordinate patient care activities and communicate effectively with healthcare providers
- Monitor patient progress and update treatment plans as necessary
- Provide education to patients and families on healthcare management
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Conduct assessments for behavioral health patients and provide appropriate interventions

*Qualifications*
- 1+ year case manager experience

- BLS (american heart association)

- Graduate of an accredited school of Nursing (Required)

*13 week contract*

*day shift - 8AM - 4:30PM*

*40 hours/week*

Job Types: Full-time, Contract, Travel nursing

Pay: $2,500.00 per week
