Purpose of Position:
The purpose of this position is to assist in obtaining all appropriate benefits, determining the type of assistance required and assist in coordinating the activities of the admission process from point of referral to completion of paperwork and transfer to the Case Manager.
Description of Required Duties and Tasks:
Essential duties and responsibilities include the following.
•Assist in the completion of the admission packets in accordance with approved policies and procedures
•Assist in maintaining complete and current social records of patients
•Assist in the assessment of the patient's physical, psychological, social, and environment status identifies appropriate interventions and initiates a plan of care based on these findings
•Participation in patient care plans and invites families to attend
•Assist in recording observations accurately and maintains the patient's medical records in accordance with local, state and federal regulations
•Assist in the support of the patients in their daily lives; encourage, listen, respect and reassure patients in all aspects of social service
•Assist in conducting individual room visits and encourage social interaction
•Assist in coordinating telephone and other communication with families and agency to carry out social needs of patients
•Establish and maintain community involvement and create a facility-like atmosphere for patients.
•Assist with the admitting, transferring and discharging of patients
•Assist in the Participation of surveys made by authorized government agencies
•Assist in the coordination of the Social Services Department and other departments to maintain quality care for patients
•Assist in the preparation of reports and recommendations for the Administrator regarding the operation of the Social Services Department
•Assist in the reviewing and revising social services care plans on a regular basis within the guidelines of state and federal regulations and facility policy
•Maintain privacy and confidentiality of records, conditions, and other information relating to patients, employees and facility
•Other duties as assigned by the Supervisor
Required Skills:
•Assist with meals as needed
•Assist with the maintenance of inventory and purchases of equipment, supplies, etc.
•Maintain a reference library of activities materials to assist the Social Services Department in meeting patients' needs, and to comply with state and federal regulations
•Perform other related duties as assigned by supervisor
Required Work Experience:
•Six (6) months experience preferred, preferably in a skilled nursing or long-term care facility, hospital, or other health care facility
•Exposure to healthcare marketing is a plus
Education Requirements:
•Associate degree preferred with an emphasis on nursing, social services, psychology, social work or related subject.
License/Certification Requirements:
•Current licensed Social Worker preferred
•Current CPR Certification
•Current State of Tennessee Driver's license
•Current Auto liability Insurance according to Company Policy
Other Requirements:
•Position may require unscheduled overtime, week-end work
•Must be willing and able to work flexible hours
•Travel required

Physical Requirements:
•Job may require frequent lifting of objects up to 40 pounds
